I ended up calling the NCPLS to ask and they had to debate this before giving me an answer.   It seems they just want a receipt and they add the extra caliber in their system but don't update your license.

can anyone that's actually done one of these confirm that they didn't add an extra entry onto your license?   I was half expecting them to print an additional line with each caliber.   I asked them about that and they insisted they just update their system and don't add an extra line.
